Guest(s) of Members Waiver

I am signing this waiver on behalf of myself and all MINOR individuals in my party listed above, including any minors for whom I am the parent or legal guardian.

ALL individuals 18 years or older MUST sign their own waiver form.

I confirm I have authority to bind all members of my party to these terms.

Guests
1. Members must accompany all guests, register their names and pay the guest fee.
2. Guests will be asked to wait at the main gate until an adult member arrives to register them.
3. Members must accompany their guests at all times in the park. It is the member’s responsibility to make sure their guests abide by all Park rules.
4. Members’ children who have established households away from their parents’ home shall not be permitted entry to the Park by using their parents’ membership.
5. Guest fee: $10.00 for each guest 3 years of age up to 64 years of age, $5.00 for guests 65 and older. Children under 3 years of age are admitted free.
6. Guest regulations may change, at the Board of Directors’ discretion, for special events.
7. Guests must complete waivers.

RELEASE & WAIVER

As additional consideration for my participation in swimming /kayaking/stand up paddleboarding (SUP)/pedal boats/fishing boats as water activities along with other on land activities offered such as but not limited to volleyball/basketball/pickleball/gaga ball at Crystal Lake Development Corp., the undersigned represents, covenants and agrees, on behalf of themselves, their heirs and assigns and any other person claiming by, under or through them as follows:

1. I acknowledge that participating in swimming/kayaking/stand up paddle boarding (SUP)/pedal boats/fishing boats as water activities and volleyball/basketball/pickleball/gaga ball/ other offerings as on land activities (“Activities”) involves risks, some of which I may not fully appreciate, and that injuries, death, property damage or other harm could occur to me or others. I acknowledge and voluntarily accept all risks of such injury, death or property damage that may occur while participating in the Activities and all associated activities regardless of whether or not caused in whole or in part by the negligence or other fault of Crystal Lake Development Corp, its agents, employees, managers, members or other guests.

2. I waive all claims and forever release and discharge Crystal Lake Development Corp, its agents, employees, managers and members for any injuries, damages, losses or claims, known and unknown, which arise from my participation in Activities associated with Crystal Lake Development Corp whether caused by Crystal Lake Development Corp, its agents, employees, managers, members or other guests.

3. I agree to indemnify and hold harmless Crystal Lake Development Corp, its agents, employees, managers and members from and against all losses, claims, liabilities, damages, cost or expenses, including but not limited to attorney fees and other dispute costs and expenses, incurred by Crystal Lake Development Corp, its agents, employees, managers, members or insurers as a result of my participation in the Activities whether or not caused by me or to me.

4.I acknowledge there is an inherent risk of various communicable diseases that exists in public settings where people are present. I agree to participate in the Activities and all associated activities at my own risk. Members/ guests are welcome to wear a mask if they choose to do so, but this is not required. Crystal Lake Development Corp will continue its normal ongoing procedures.

5. I have reviewed the rules & policies for Crystal Lake Development Corp., members & guests for the use of kayaks/stand up paddle boarding (SUP)/pedal boats/fishing boats and facilities. I agree to adhere to the stated rules & policies for Crystal Park Development Corp..

6. I have carefully reviewed this guest RELEASE AND WAIVER OF LIABILITY, understand its provisions and execute it voluntarily.
